Functional Palatoplasty era
In functional palatoplasty during 1964–1977, surgeons prioritized restoring dynamic velopharyngeal closure by reconstructing the levator veli palatini. Pierre Tessier advanced flap-based, muscle-centered reconstruction within a broader craniofacial framework to emphasize functional restoration. John H. Mulliken and colleagues promoted multidisciplinary cleft care and standardized planning, aligning surgical timing with orthodontic and prosthetic inputs. Robert H. Grabb and Peter Kaban contributed muscle reorientation techniques and the use of vascularized adjuncts, including periosteal and lingual flaps, to improve wound stability and functional outcomes.
